J is 33 years old and lives in Asunción, Paraguay.
She has no previous history of diabetes, hypertension, or
dyslipidemia, but she has been obese since her teenage years.
She is 5 feet 5 inches (1.65 metres) tall and weighs 279 lb
(127 kg).
There is a history of diabetes in her family and her 17-year
old brother has Type 1 diabetes which was diagnosed at the
age of 10. He is treated with multiple insulin injections.
Her 29-year old non-obese sister was diagnosed with diabetes
last year, and is doing well on twice-daily premixed insulin
treatment. Her parents are not diabetic. She was referred
to specialist care following a random blood sample test.
J has no outward signs of hyperglycaemia but a random sample
test revealed a capillary blood glucose concentration of 234
mg/dl (13 mmol/l). She is not currently receiving any medication.
