Detailed history taking revealed that the patient has a strong family history of diabetes: her mother was diagnosed with diabetes during her last pregnancy in her late 30s, but received treatment only in her 40s. She developed heart and kidney disease as a result of her diabetes. B has 3 sisters and 1 young brother. Her eldest sister became blind at the age of 28, presenting with vitreous haemorrhage, and has been treated with insulin since then. Her younger sister was diagnosed incidentally with diabetes at the age of 12 when she underwent a nasal polyp operation, and has been treated with insulin since. Her maternal grandmother also had diabetes at the age of 60 years and died of unknown causes in her late 60s.

B is a non-smoker and eats out often. She commutes to work daily but does not take regular exercise. She complies well with her diabetes therapy.
